Care Provider Registration Requirements
Nurses (RNs and LPNs)
- RN or LPN license
- Level 2 Background screening on file with AHCA
- Valid CPR card from American Heart Association or American Red Cross
- Free of communicable disease statement dated within 6 months
- 1 hour of HIV/AIDS Certificate
- 30 hour Infusion Therapy Certificate (LPN only)
- Professional Liability Insurance - recommended
- Current FL Driver's License
- Current Automobile Insurance
- Current Vehicle Registration
Home Health Aides and Certified Nursing Assistants
- CNA License in Florida (or HHA certificate - 40 hour minimum)
- Level II Background Screening on file with AHCA
- Valid CPR Card through American Heart Association or American Red Cross
- Free of Communicable Diseases (dated within the last 6 months)
- HIV/AIDS Certificate - 1 Hour
- Alzheimer's Training Certificate - 1 Hour
- Self-Administration of Medication Certificate - 2 Hours
- Professional Liability Insurance - recommended
- Driver's License
- Social Security Card
- Auto Insurance & Vehicle Registration - You must have your own transportation
Requirements to Register as a Homemaker, Companion, or Sitter
- Level 2 background screening on file with AHCA – State of Florida
- Two work references
- Social Security Card
- Professional Liability Insurance - recommended
- Current Florida Driver’s License
- Current Auto Insurance
- Current Vehicle Registration – You must have your own transportation
